OUTCASTS DFW BASS CLUB AGREEMENT AND RELEASE
I, __________________________________, desire to participate in the activities sponsored or conducted by the Outcasts DFW Bass Club.
I agree to abide by all of the rules and regulations, now or hereafter adopted by the Club or by its representatives, of all activities.
I further agree to return in good condition, ordinary wear and tear accepted, any and all equipment which has been or may be issued to me by the Club as soon as an activity has been concluded.
In exchange for my being permitted to participate in the Club's activities, I, for myself, my heirs, executors, administrators and assigns, do hereby waive, release and forever discharge any and all claim demands, suits or causes of action against the Bass Club, its Officers and respective successors and assigns, for any injuries, accidents, illness, losses or damages which I may incur as a result, either directly or indirectly, of my participation in any Club.
I have full knowledge of the risk or possibility of accident, injury or illness to me or to others by reason of my participation in these activities, and I voluntarily assume and accept such risks as my sole responsibility, knowing that the Club will rely upon this agreement and release.
This release is complete and final and contains the entire agreement between me and the Bass Club. No other written or oral agreements affecting this release exist.
